TERMS OF USE & PRIVACY POLICY
PLEASE READ THIS AGREEMENT CAREFULLY. THIS AGREEMENT GOVERNS YOUR USE OF THE WEB SITE. IF YOU DO NOT AGREE TO THE TERMS OF THIS AGREEMENT OR ANY REVISED VERSION OF THIS AGREEMENT, PLEASE DO NOT USE THIS WEB SITE.
DISCLOSURE OF MEDICAL INFORMATION
The Company takes seriously the privacy of your medical records and personal information. Accordingly, we have taken all reasonable restrictions necessary to protect your confidentiality in accordance with all applicable laws.
In addition, you authorize the Company and their respective physicians, employees, contractors, business associates and/or agents to access, review, research, analyze, discuss and copy your medical and personal information as may be necessary to provide the second opinion you request. The use of your medical information will not exceed this stated purpose.
PAYMENT OF FEES
You acknowledge that you or any authorized individual acting on your behalf and who is requesting the second opinion report is responsible for all fees charged by Consult medical services for the services hereunder (including, but not limited to, fees for professional second opinion services). You understand that you must pay these fees in advance, and that it is your responsibility to pursue any third-party insurance reimbursement at your own expense, if any. You have been informed that you have the option to withdraw consent for this second opinion radiology report at any time. However, if you withdraw your consent after payment is made, you will remain responsible for the full amount. No money will be refunded after services are issued.
RELEASE OF LIABILITY
You further release and hold harmless Consult medical services and its/their respective physicians, employees, contractors, business associates and/or agents for any harm, claim, injury or damages of any kind including, but not limited to, compensatory, direct, indirect or consequential damage, directly or indirectly, as result of any and all uses of the services hereunder and any review, interpretation or analysis or and/or reliance on any and all second opinion reports. Although every effort will be made to generate a second opinion report in reasonably timely fashion, Consult medical services makes no representation or warranty as to the timeframe in which such second opinion radiology report will be generated.
WE DO NOT PROVIDE MEDICAL ADVICE
The Information that you obtain or receive from Consult medical services and its employees, contractors, partners, sponsors, advertisers, licensors or otherwise on the Site is for informational and scheduling purposes only.
THE INFORMATION PROVIDED ON THE SITE IS NOT INTENDED AS A SUBSTITUTE FOR, NOR DOES IT REPLACE, PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, OR TREATMENT. DO NOT DISREGARD, AVOID OR DELAY OBTAINING MEDICAL ADVICE FROM A QUALIFIED HEALTH CARE PROFESSIONAL BECAUSE OF SOMETHING YOU MAY HAVE READ ON THE SITE. DO NOT USE THE SITE FOR EMERGENCY MEDICAL NEEDS.YOUR USE OF INFORMATION PROVIDED ON THE SITE IS SOLELY AT YOUR OWN RISK. NOTHING STATED OR POSTED ON THE SITE OR AVAILABLE THROUGH ANY SERVICES IS INTENDED TO BE, AND MUST NOT BE TAKEN TO BE, THE PRACTICE OF MEDICINE OR THE PROVISION OF MEDICAL CARE.
We do not recommend or endorse any specific tests, physicians, procedures, opinions, or other information that may appear on the Site. If you rely on any of the Information provided by the Site, you do so solely at your own risk.
LIMITATIONS OF THE SECOND OPINION MEDICAL SERVICE
- The physician rendering the Second Medical Opinion report will not have the benefit of examining you in person, the ability to order additional tests, or have any information beyond what you provide. Since the medical experts will not personally examine you or order additional tests, it is not a medical diagnosis. The report is intended to provide you with information to supplement the information you have already received from your treating physicians. The information contained in the Second Medical Opinion report shall not be used to substitute for your physician’s recommendations.
- You should discuss the Report with your own doctors, who are responsible for your care. Consult medical services will not evaluate or make any recommendations regarding the content of the report on receipt from the third party provider, and shall not be liable for any loss howsoever arising out of the preparation, delivery and the content of such report including your interpretation and subsequent actions taken by you in reliance thereon. For the avoidance of doubt, Consult medical services or its affiliates shall have no liability whatsoever for any loss, liability, claim, expense or damage suffered by the Applicant and/or the Patient, resulting from any advice or services provided by the Suppliers, or any acts or omissions, or incompliance of the Suppliers or any sub-contractor, service provider or any third party acting on the Supplier’s behalf in the provision of the Services, 3. The Second Medical Opinion service is not applicable to cases of first aid, accidents and ICU related critical illness. No prescription will be provided.
